Job Description

Summary:

Workforce Group (WFG) is seeking a compassionate and experienced Advanced Practice Registered Nurse (APRN) to support formerly homeless individuals with severe and persistent mental illnesses. This role will provide direct clinical care, medication management, and behavioral health support, working as part of a multidisciplinary team to ensure comprehensive care coordination. The APRN will report directly to the Program Lead and oversee the interdisciplinary team, ensuring high-quality service delivery that promotes housing stability, mental health recovery, and overall well-being.

Location: Southeastern Louisiana, including but not limited to (Slidell, New Orleans, and Lafayette areas)

Duties and Responsibilities:

• Conduct comprehensive psychiatric and medical assessments for individuals with severe and persistent mental illnesses (SPMI) and co-occurring conditions.
• Provide direct care, including medication management, treatment planning, and crisis intervention.
• Develop and implement individualized treatment plans in coordination with the interdisciplinary team, ensuring integrated healthcare approaches.
• Work closely with case managers, social workers, peer support specialists, and clinicians to address both medical and psychosocial needs.
• Serve as the clinical lead, offering guidance and supervision to team members in best practices for mental health care and homeless service delivery.
• Collaborate with community agencies, hospitals, and behavioral health providers to facilitate continuity of care and access to specialized services.
• Participate in multidisciplinary team meetings, case consultations, and service planning to ensure holistic client care.
• Conduct home visits to assess client well-being, support treatment adherence, and provide education on health management strategies.
• Be available for on-call responsibilities as needed to provide urgent clinical support and crisis intervention.
• Maintain accurate documentation and compliance with all program guidelines, licensing requirements, and state regulations.
• Provide training and support to team members on mental health interventions, trauma-informed care, and crisis de-escalation techniques.
• Travel throughout the region as needed to ensure service delivery, client support, and team oversight.
• Perform other job-related duties as assigned.

Qualifications, Knowledge, Skills, and Abilities: Must hold a valid Advanced Practice Registered Nurse (APRN) license in the state of Louisiana. Must possess current Basic Life Support (BLS), Advance Cardiac Life Support (ACLS) Certifications. Minimum of two (2) years of experience working with homeless populations and individuals with severe and persistent mental illnesses (SPMI). Experience in psychiatric care, behavioral health, substance use disorders, and trauma-informed care.

• Strong leadership and team oversight skills to guide interdisciplinary collaboration.
• Expertise in psychiatric care, behavioral health, substance use disorders, and trauma-informed care.
• Ability to conduct home visits and provide care in non-traditional settings, including shelters and community-based environments.
• Extensive knowledge of mental health treatment models, medication management, and crisis intervention strategies, and de-escalation techniques.
• Strong communication, interpersonal, and advocacy skills to engage with both clients and community partners.
• Ability to work independently while also collaborating within a multidisciplinary team
• Willingness to travel throughout Southeastern Louisiana as needed for service coordination and home visits.
