We are seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our skilled nursing facility team in San Jose, CA.
Responsibilities:
- Assist in evaluating and assessing patients' needs
- Collaborate on individualized treatment plans
- Provide hands-on therapy and interventions
- Teach skills to enhance patient independence
- Monitor progress and adjust treatment plans
- Document patient progress accurately
- Coordinate care with other healthcare professionals
Qualifications:
- Experience in Skilled Nursing or Long-Term Care preferred
- Certification as an occupational therapy assistant
- Interest in working with the geriatric population
- Strong communication and teamwork skills
