MTM Pharmacist Highlights

• The pay for this position is $54.05 per hour paid biweekly.
• Monday - Friday, 9:00AM – 5:00PM CT (Training will be 8:00a-4:30p CT)
• This position is a contract to hire.

Responsibilities

• The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM).
• Effective delivery of this CVS Health telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems.
• This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers.
• The MTM Pharmacist interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health-related goals and help them on their path to better health.
• Pharmacists will also be trained to use the innovative skill of behavioral modification and interviewing, which are key drivers to positively impact patient health outcomes.

Requirements

• Bachelor’s degree in pharmacy or PharmD from an accredited school of Pharmacy
• Applicant must reside and have an active pharmacist license in either HI, TX, IL, FL, or PA

Preferred:

• Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
• Basic computer skills and proficiency with Microsoft Office products.
• Strong organizational skill, interpersonal skills, and detail orientated.
• Experience maintaining focus and meeting goals in a work from home environment.
• Previous call center experience
