Summary:
Facilitates Hospice and Palliative Care referrals from all sources and obtains reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program. The position assumes dual accountability for Palliative Care and Hospice referrals.

Qualifications:*Associates Degree or Diploma in Nursing minimum
*New Mexico RN License required.
*1 year of hospice experience required
*1 year of acute care experience strongly preferred.
*Current drivers license and auto insurance.
*Basic typing competency.
