SPECIFIC REQUIREMENTS:
The Admissions Liaison will assist in processing referrals and admissions to the assigned facility.
The Admissions Liaison will assess patients’ needs for admission consideration, and where appropriate, utilization of alternatives.
The Admissions Liaison will work constructively to ensure compliance within assigned facilities according to policies and procedures.
The Admissions Liaison will ensure compliance with State and/or Federal regulations concerning the waiting list, inquiry book, daily census and pre-admission screenings.
The Admissions Liaison will complete all pre-admission and admission documents as assigned.
Knows and complies with facility Privacy Practices policies and procedures and is able to and educates residents and families in the same.
The Admissions Liaison will provide back-up support to the assigned Admission Department.
The Admissions Liaison provides written reports and recommendations following consultation visits made to each facility as directed by the Corporate Director of Admissions.
The Admissions Liaison will attend quarterly Admissions Meetings.
The Admissions Liaison will attend mandatory meetings as required.
The Admissions Liaison serves as a resource for facility Admission staff and Administrators, providing ongoing information, consultation and training regarding the referral tracking system.
The Admissions Liaison receives and reviews the facility census report on a daily basis and is responsible for the census mix and budget for buildings as assigned.
EDUCATION/EXPERIENCE:
RN, LPN preferred. Minimum of two (2) years experience in an Admissions Coordination position in a skilled nursing facility. Must have demonstrated capabilities for management, administration, and supervision. Demonstrated leadership qualities. Must have thorough knowledge of local, state, and federal rules and regulations as they pertain to Admissions.
