St. Croix health is currently seeking a full-time Advanced Practice Provider to join our Lindstrom Clinic Family Practice team.

The Advanced Practice Provider is responsible for the care, treatment and patient education for patients and their families in the clinic settings. The Advanced Practice Provider will perform evaluations, review medical history and conduct physicals, and order appropriate diagnostic services.

Essential Duties:

1. Provide patients with continuity of care at a professionally recognized level of quality and efficiency. Ensures proper injury care and disease prevention, diagnosis, treatment, and recovery. Prescribes and administers medications, orders routine vaccinations, and provides advice regarding personal health, hygiene, ADL capabilities; Requests necessary tests and follow-up visits and refers patients to specialists, as necessary.

2. Complete documentation and in-basket needs (results) in a timely manner.

3. Aid clinic staff with leadership and education.

Education & Licensure:

• Licensed by the State of Minnesota and Wisconsin
• Certified by the NCCPA or APNP
• Certified on Basic Cardiac Life Support
• Completion of a physician assistant program accredited by the Accreditation Review on Education for the Physician Assistant (ARC-PA) or completion of a nurse practitioner program accredited through the American Board of Nursing Specialties

Experience:

• Family Practice experience preferred.
