Facility Description

Farmington Health Services (AKA Trinity Campus) is a five star rated senior living community.

  • Trinity Terrace provides independent and assisted living services.

Position Description

  • Adhere to, support and promote St. Francis Health Services’ Mission and Core Values, philosophy, goals and objectives while conducting company business.
  • Work with the Director of Home Care to ensure that Home Care is in full compliance with federal, state, local law and company policies and procedures.
  • Recommend development of and changes to policies and procedures.
  • Monitor standards of care, service and productivity, ensuring a high quality of life for tenants.
  • Communicate, as appropriate, with staff, physicians, referral sources and tenants.
  • Establish informational systems which aid employees to fully understand Home Care’s mission, philosophy and operations.
  • Provide tours to prospective tenants and actively market the care center.
  • Provide in-services for all staff and ensure all staff meets in-service requirements.
  • Represent Home Care in dealings with outside agencies, including governmental agencies and third party payers and at various meetings as necessary.
  • Verify and approve Home Care employees’ electronic timecards each pay period according to company policy and procedures.
  • Disciplines personnel as requested or as may become necessary and ensures any disciplinary action is administered fairly.
  • Ensure that all care is provided in privacy, as appropriate, and to knock before entering a tenant’s residence.
  • Ensure service satisfaction through tenant visits and other measures as appropriate.
  • Responsible for investigating and resolving all tenant complaints.
  • Complete move-in paperwork for tenants (i.e. lease agreement, apartment inspection, tenant handbook and consumer guide).
  • Utilize listening skills that indicate understanding and promote accurate interpretation of others' concerns, motives and feelings.
  • Work toward resolution of interpersonal conflicts as they arise.

Qualifications

  • Experience in Healthcare or Housing/rental management preferred.
  • Direct care experience a plus

Job Summary:

The primary responsibility of your job position is in the management and oversight of the day-to-day functions of the Assisted Living in accordance with current federal, state, and local standards, guidelines, and regulations that govern Assisted Living to ensure that the highest degree of quality of care is maintained at all times.

Job Requirements:

  • Must possess a current, unencumbered, active license in Minnesota as a licensed Assisted Living Director or commit to obtaining a license within one year of employment.
  • Must be knowledgeable of computer systems, system applications, and other office equipment.
  • Two years’ experience in home health and/or housing.
  • At a minimum must have one year of management experience.
