Associate Director of Care Management

Position Reports To: Sr. Director of Care Management

Corewood Care Management is a recognized aging life care management company and home care company helping families in the DVM area. Key Tasks, Duties, and Responsibilities

Operations Management

  • Responsible for designing and implementing effective business strategies to grow the organization through organic growth as well as through ongoing relationship building and nurturing of business partners, family and employees.
  • Implement, manage and evaluate current operational processes and procedures in accordance with the standards and procedures set out by the organization.
  • Assist in developing strategies and implementation plans to improve and standardize all aspects of operations.
  • Develop, write and implement policies and procedures to scale the company.
  • Creates a positive and healthy work environment for all staff and ensures policies and procedures align and promote the company's culture, mission and vision; leading by example with transparent and professional communication.
  • Establishes a managerial climate that encourages teamwork and promotes collaboration among all team members.
  • Promotes teamwork and greater engagement among on-site and in the field team members through effective mentorship and performance management practices
  • Assesses employees on an ongoing basis to ascertain their understanding of policies and procedures.
  • Provides guidance and leadership over weekly team meetings, case review and relevant activities pertaining to key performance metrics.
  • Maintains two-way communication with employees and fair administration of personnel policies.
  • Establish metrics for key areas of measurement within operations and hold teams accountable to achieve or exceed these goals.
  • Ensures competent and mission-driven staff are hired and finds ways to continually offer developmental opportunities creating an environment of teamwork and growth-minded individuals.

Administrative Management

  • Effectively collaborates with teams across functions and needs
  • Ensures tasks completion across multiple operational functions.
  • Provide oversight and guidance to CM/ACM/WP personnel.
  • Provides direction and leadership across all operational areas, including:
  • Oversees care management operations
  • Assigns CM/ACM/Wp to cases as deemed appropriate,
  • Provides scheduling oversight
  • Aware of CM/ACM/WP hours and schedule and provides updates, information, and suggestions for effective decision making and client/employee matching.
  • Reviews overtime considerations4 and ensures CM/ACM/WP are working correct hours.
  • Reviews case notes for completeness, consistency, concerns, issues and review.
  • Manages prospect list and ensures database is current
  • Oversees CM recruiting
  • Maintains and evaluates compliance with applicable laws and regulations.
  • Hiring and training new CM staff
  • Provides oversight on CM filing systems for accuracy and compliance
  • Maintains regulatory and compliance measures
  • Addresses client or CM complaints, issues or concerns
  • Cohesively communicates with CM/ACM/WP to ensure strategic resolutions are applied
  • Provides guidance and mentorship
  • critically and strategically applies resolutions for stressful or emergency situations with both CM/ACM/WP and clients across multiple functions of operations.
  • Understands and conducts themselves with a sense of urgency
  • Makes strategic and comprehensive decisions and provides support to decision making
  • Uses critical thinking skills to address client or field staff conflicts.
  • Creates and/or assists in creation of company policies and procedures and ensures implementation.
  • Performs any additional tasks as required

Job Types: Full-time, Part-time

Pay: $60,000.00 - $85,000.00 per year

Education:

  • Bachelor's (Preferred)

Experience:

  • Social work: 3 years (Preferred)

License/Certification:
