Behavioral Health Therapist/Social Worker Supervisor
What is a PACE program?
  • The Program of All-Inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Behavioral Health Therapist/Social Work Supervisor Responsibilities Include But Are Not Limited To:
  • Provides psychotherapy services to PACE participants based on assessed need.
  • Creates treatment plans that reflect patient’s values, goals, and health. Incorporates feedback from the Interdisciplinary Team in the treatment plan.
  • Understands legal issues that affect treatment, including but not limited to adult abuse, duty to warn, recipient rights policies and procedures, alternative treatment orders, and the commitment process for involuntary psychiatric hospitalization.
  • Conduct home visits as needed to meet the mental health needs of PACE participants in the community.
  • Encourages effective relationships among staff geared to team building and maintenance of a cohesive team. Organize and run regular social work team meetings.
  • Participates in Program and Policy development of the Social Work department.
  • Interprets agency policy and procedures to staff and serve as a liaison, bringing staff concerns and needs to the Center Manager as needed.

Qualifications & Requirements:
Education: Master’s Degree in Social Work and must be licensed in the state of Michigan.
Experience: 2 years experience in a health related area. 2 years experience in supervision. Minimum of 1 year experience working with the frail elderly population.
Licensure, Registry or Certification Required: LMSW, valid MI Drivers License and vehicle, CPR certification.
Special Training: Individual therapy skills for geriatric participants, analytical thinking and problem solving abilities. Includes some home isits to private homes of PACE Central Michigan participants. Meet a standardized set of competencies for the specific position description established by PACE Central Michigan and approved by CMS before working independently.
