GENERAL DESCRIPTION: The Case Manager is responsible for providing case management services for the UC referred by the Office of Refugee Resettlement (ORR).
MAJOR DUTIES AND RESPONSIBILITIES:
- Responsible for assessing the needs of each UC in care.
- Works with the Program Director and Clinician to match UC’s with foster homes.
- Developing an Individual Service Plan.
- Screening for human trafficking concerns.
- Facilitating the timely release or discharge of the UC.
- Documenting the provision of services in each UC’s case file.
- Maintaining direct contact with each UC and, to the extent possible, with the UC’s family and potential sponsors.
- Conducting interviews and background checks on sponsors to ensure UC are discharged into safe environments.
QUALIFICATIONS: The Case Manager is required to have a Bachelor’s Degree in the behavioral sciences, human services, or social services fields or, alternatively, a minimum of an Associate’s Degree and three years of progressive employment experience in the aforementioned fields that demonstrates case management experience. Bilingual (Spanish-speaking) required.
The Case Manager is required to be able to operate a personal computer with proficiency in the use of web-based platforms and Microsoft Word. The applicant must pass a national background check conducted by fingerprints, a state police background check, a CT DCF protective service background check, and a motor vehicle check.
