Bilingual Service Navigator (English/Spanish)-Preferred
Location: MCS 505 E. Columbus Ave., Springfield, MA
Responsibilities: Service Navigators will be cross-trained to work in the 3 programs listed above.
The service navigator will work with a variety of families including but not limited to families who have a member living in their home who has complex medical challenges and needs; families who need services to prevent out of home placement and families who are seeking support to navigate the DDS eligibility application process. Supports include but are not limited to, case management, accessing generic services, assessing needs, developing Family Support Plans, and providing information and referral services.
Must have valid driver’s license and dependable vehicle.
Hours: Position: 40 hrs./week Status: Full time
- Maintains quality of programming through adherence to agency standards and DDS regulations.
- Service Navigation for the individual/family
- Where applicable, hiring and supervising direct care staff
- Work closely with DDS Service Coordinators
- Outreach to generic services and collaboration with other state agencies and service providers
- Maintain accurate financial records
- Ensure that all documentation is completed on a timely basis, and other duties as required
- Work with school districts to provide continuity of services from school to home.
QUALIFICATIONS: BS or BA in related field with one-year experience preferred. A Minimum of High School Diploma with 3 years of experience in the field of Human Services. Bilingual (English/Spanish) a plus.
