Responsibilities:

• Assist seniors through an initial home visit(s) and on-going assessment of the client’s caregiver needs.
• Develop a tailored care plan to meet these needs and continuously evaluate and modify to promote safety, quality and client satisfaction.
• Communicate with all involved parties such as family, vendors, and MySolutions staff.
• Convey the relevant details of the service plan and staffing needs, preferences and special requests to the MySolutions scheduling staff.
• Complete documentation required under state license within the defined timeline. Introduce and orientate caregivers to the client, client’s home and clients care plan.
• Investigate and complete incident reports for any incidents that happen during the MySolutions service hours.
• Follow and promote confidentiality involving client and family interests.
• Monitor the care plan, adjusting the plan as needed. Provide support, reassurance and education while focusing on problem solving to achieve and maintain both client satisfaction, as well as MySolutions caregiver satisfaction.
• Ensure respect of clients’ possessions and home environment.
• Contribute and participate caregiver quarterly meetings, newsletter and ongoing training of MySolutions staff as needed.
