Community Health Worker or Care Coordinator - Full Time

Compass Health is currently looking for a Community Health Worker or a Care Coordinator for our Outpatient Care Coordination Program!

The Care Coordinator/Community Health Worker is responsible for ensuring an integrated, whole person health approach to address the health and well-being of individuals served. The Care Coordinator organizes care activities and facilitates sharing of information among all care team members (client, natural supports, healthcare providers, social service providers, other support systems) to ensure appropriate and effective care. Services delivered by the Care Coordinator are recovery-focused and include but are not limited to care coordination, care management, resource identification and system navigation, health promotion, transitional support, and identification and development of supports and resources.


What you will be performing

  • Ensure services delivered focus on client’s health and well-being using a whole person health perspective.
  • Work collaboratively and effectively with providers and systems to reduce barriers and inequities related to health and cultural needs of the individual being served as well as Compass Health clients in general.
  • Actively engage clients primarily through community-based, face-to-face encounters.
  • Conduct comprehensive health screening inclusive of physical health, behavioral health, and social service needs.
  • Consult and collaborate with client, natural supports, and multidisciplinary care team, inclusive of providers external to Compass Health care team members, to develop an individualized health action plan to address identified needs.
  • Identify, refer, and facilitate access to relevant care, services, and resources to support whole person health goals.
  • Provide system navigation to ensure necessary care and supports are secured.
  • Provide client education regarding conditions, treatment options, and resources to promote client activation and self-management.

What you will be carrying along

  • Minimum High School Diploma or equivalent required. BA/BS/BSW Degree in a healthcare or social services related field preferred. Specific programs may require a degree in a behavioral health related field.
  • Experience in providing care coordination or case management services preferred.
  • Demonstrated knowledge of behavioral and physical health issues, or interest and ability to learn.
  • Individuals who do not already hold a license or associate license through the Washington State Department of Health must submit Agency Affiliated Counselor application upon hire and be approved by the DOH.
  • Current First Aid & CPR certification, or ability to obtain within 90 days of hire.
  • Valid Washington State Driver’s License including vehicle and appropriate insurance, if applicable.
  • Bilingual/bicultural encouraged to apply.
