Description:Summary:
The Care Coordinator operates as part of the Care Team, navigates all patients through the system acting as patient advocate, working collaboratively with the providers, clinical staff to support patients with chronic conditions and complex needs according to guidelines set forth by FQHC.
Essential Duties & Responsibilities:
- Meets with all clients to ensure appropriate insurance coverage. Gathers all necessary information for insurance activation.
- Screening of social determinants and link to appropriate services.
- Communicates with specialty providers to ensure treatment.
- Facilitates claims appeal process to obtain insurance authorizations.
- Provides ongoing support throughout referral process.
- Works collaboratively with the treatment team to ensure best client outcome.
- Participates as a member of an interdisciplinary treatment team in the assessment, development, implementation and evaluation of client treatment or health/mental health care plans.
- Collect necessary data related to client health information and outcomes.
- Performs all other duties as required.
- *All agency staff are required to attend all mandatory department/agency meetings and trainings**
Requirements:Education &/Or Experience:
High school graduate with a minimum of 5 years’ experience in a public health care setting or Medical Assistant training with two years of Public Health Care experience required.
