Care Manager Job Description


Educational Requirements
: Masters level social worker.

PER DIEM ROLE, based out in the Boston Area


Experience
: Experience working with adults, families, mental health services, case management, comprehensive assessments, guardianships and or conservatorships.


General Job Description: Care Managers are professional practitioners whose purpose is the development, advancement, and promotion of humane and dignified social, psychological, and health care for the client and their families through the delivery of concrete services, consulting, advocacy, and coordination of overall care needs by qualified providers. Care Managers comprehensively assess the multifaceted needs of a client and the client’s family caregiving system. The Care Manager’s goal is to arrive at a solution that allows maximum decision making autonomy for the client and for the other persons involved with or affected by these care needs.


Duties (including but not limited to):

  • Conduct objective assessments
  • Resource location and coordination
  • Coordination of overall care needs
  • Liaison with family and health care providers
  • Crisis intervention
  • Supportive counseling
  • Preparing reports
  • Mediation
  • Accompany elder to Emergency Room/doctor appointments if necessary
  • Follow through on care plans
  • Discharge planning
  • Support groups/community education
  • Peer counseling
  • Family meetings
  • Familiarity with long term care facilities
  • Assist family caregivers who are at a distance from the elder,
  • Assisting elders who have been the sole caregiver for their adult children with MRDD.
  • Completing continuing education requirements


Hours
: Per Diem role. Hours vary and are case load dependent.

The Overlook is a non-profit organization operating since 1911 and offering a full continuum of care and services for older adults in MA. The organization embraces its rich history and heritage, owning and managing 218 Independent Living Apartment Homes and Cottages, 28 Enhanced Care Private Suites, a 14- Suite Assisted Living Memory Care neighborhood, 27-unit Short-term Post-Acute Rehabilitation, 112-bed Long Term Care Skilled Nursing, as well as Home Health, Hospice, Palliative Care, Private Duty and Care Management divisions.

The Overlook and its 3 subsidiaries are based in Charlton, MA, with satellite home health and hospice offices across the state. The organization employs approximately 600 talented and compassionate Team Members and serves 1,500+ Residents/Patients.
