GENERAL DESCRIPTION: The UC Case Aide is responsible for supporting the UC Case Managers in goals towards the safe and timely release of youth and once their cases have been approved for release to facilitate the release process.
MAJOR RESPONSIBILITIES:
  • Serve as support for Case Managers in administrative tasks and phone calling, preparing documentation, scanning materials and other document preparation for release to sponsors.
  • Assist in the release of all youth within 24 hours of case approval
  • Coordinate with Case Management Team to facilitate flight arrangements, communication of transportation details, exit packet documentation, and program release from the UC Portal
  • Responsible for verifying biographical information prior to unification planning
  • Plan reunification collaboratively with the Case Manager and the Sponsor
  • Collaborating with airlines to reserve flights for youth and staff as needed
  • Preparing Exit Packet for youth, including the collection of identifying documents, medical records, legal resources, educational assessments, and youth’s possessions in care
  • Assist Case Managers to complete required documentation as it relates to unification
  • Process release of the youth in the UC Portal systems
  • Provide copies of all documentation provided to youth at the time of exit and file in the youth’s case file
  • Responsible for assisting the case managers in the completion of UC Portal documentation and maintenance of UC paper and electronic records as it relates to unification.
  • Communicate with Director of UC and Youth Care Supervisors to provide staff escorts for unification of youth under 13
  • Complete Safety and Well Being Follow up Calls 30 days after UC release
  • Responsible for continued professional development through attendance at in service seminars, participation in conferences and workshops and/or continued study at higher education institutions.
  • Attend all mandatory training as scheduled, including but not limited to New Hire Orientation, Risking Connections, TCI certifications and CPR/First Aid certifications and must complete annually all required O.R.R (Office of Refugee Resettlement) training.
  • Attend and participate in required supervisory and team meetings.
  • Adheres to agency policies and procedures; follows and supports agency mission, vision and values including participating in internal surveys and data collection for continuous improvement initiatives.
  • Performs other duties and assumes other responsibilities at the request of the Program Director, Case Managers and Assistant Director.
QUALIFICATIONS: Associates degree in Psychology, Human Services or another related field required; Bachelor's Degree or Bachelors in Social Work preferred. Related experience in Child Welfare and/or case management experience. Ability to read, write, and speak English and Spanish. Must be able to work a flexible schedule including various hours, nights, weekends, holidays, and subject to call hours. Must have a current, valid driver’s license per Agency guidelines. Travel required. Ability to work with an economically and culturally diverse population.
  • Excellent written and oral communication skills including ability to build relationships and rapidly engage youth and families.
  • Capability to coordinate and plan travel for visits and appointments in efficient manner.
  • Ability to organize work effectively, conceptualize, prioritize, and exercise independent judgment based on an understanding of internal and external organizational policies and practices including completing all state mandated and agency reports in a timely manner.
  • Computer literacy including but not limited to MS Office suite and other database programs.
  • Ability to develop and maintain collaborative working relationships with internal and external stakeholders.
  • Established Interpersonal skills including ability to listen, act as a role model, and build therapeutic relationships.
  • Ability to assess situations and resolve multiple conflicts simultaneously.
  • Handles confidential and sensitive information with extreme discretion, demonstrated knowledge of HIPAA regulations.
  • Demonstrated organizational, time management and prioritization skills required.
  • Ability to work independently as well as in a team environment in a fast-paced culture; must be able to adapt to a changing environment.
REPORTS TO: The Case Aide is supervised and accountable to the Assistant Director.
