Case Manager Job at The Family Health Centers
The Family Health Centers is the largest physician owned family medicine practice in Western North Carolina. We see patients in Asheville, Arden and Hominy Valley. We offer a range of services including lab, X-Ray, bone density, etc.
Case Manager duties include:
- Facilitates chronic care management in collaboration with the clinical staff.
- Provides Medicare Annual Wellness visits.
- Provides referrals of community resources for patients.
- Understands value-based care and incorporates concepts in day-to-day patient care.
- Participates in home visits when necessary.
- Provides floor nurse support when staffing levels require additional resources.
- Documents necessary patient care items in the E.H.R.
- Other related duties as assigned.
Case Manager Requirements
- Electronic Medical Records
- RN or LPN
- Minimum 5 years case management experience in an organizational setting is required in a hospital, ACO or CIN
