Role and Responsibilities

  • Provide case management and coordination of services to assigned UC youth and families.
  • Prepare and maintain case records of assigned UC youth and families. Ensure that records meet YFT policy, Virginia Licensing, Counsel on Accreditation (COA), Office of Refugee Resettlement and Joint Commission Standards.
  • As the treatment team coordinator, develop, implement, evaluate and revise all treatment plans of assigned UC youth and prepare written reports in a timely manner.
  • Lead and direct team meetings, participate in organizational meetings, court proceedings, multidisciplinary reviews and other meetings as necessary.
  • Participation in Evaluation Team decisions.
  • Provide consultation, coaching and training to assigned foster families.
  • Maintain direct contact with each assigned UC youth at least once a week to discuss reunification options.
  • Ensure planning process is oriented toward safe family reunification, preparation for return to the country of origin, transfer to a higher level of care or less restrictive setting, or transition to an alternative living arrangement once legal immigration status has been obtained.
  • Maintain constant communication with the UCs biological family as well as the legal service provider.
  • Consult on a regular basis with UCs therapist, educational staff, and foster parents to ensure that the UC is receiving optimal services and to review and discuss any issues or concerns.
  • Counsel UC youth and foster families in preparation for placement and discharge.
  • Provide after-hours on-call support, along with working evening and weekends as required. Ensure timely notification of appropriate individuals regarding serious incidents and medical emergencies.
  • Attendance at all training mandated by YFT.
  • All duties and responsibilities will support meeting the structured program of care outlined in DBHDS and DSS state standards (DBHDS: 12VAC 35-46-800; DSS: 22 VAC 40-151-720; 22 VAC 40-131-130).

Required Trainings: P.R.I.D.E. Training, First Aid/ CPR; Handle With Care: Standard Precautions; Emergency Preparedness; Shaken Baby; Mandatory Reporting; and 25 hours of job specific related training.

Minimum Qualifications:*

  • Master’s degree in human service field with one year of experience providing casework services to children and families.
  • Baccalaureate degree in a human service field with one year of experience providing casework services to children and families.
  • Baccalaureate degree in any field plus two years’ experience providing casework services to children and families.
  • Fluent in English and Spanish
  • Writing Proficiency in English and Spanish
  • Must have a valid driver’s license and driving record within guidelines established by YFT insurance underwriters.
