PROGRAM: Hill Street/Arioli– Licensed Level III Facility

POSITION: Case Manager II

Description–Provide Service Planning & Coordination to individuals with developmental disabilities living in two Licensed Level III homes so they may gain access to services and supports in order to achieve successful outcomes. Examples of service provision includes, but is not limited to, developing and implementing a person-centered plan, coordinating medical and clinical services, and establishing and maintaining a case record. This position includes the following job responsibilities:

Facilitates/Oversees/or Assures:

  • Staff training in all areas needed to provide quality support and to assure compliance with all overseeing agencies.
  • Health and safety of each resident.
  • Personalized growth and choice for each resident.
  • Compliance with all licensing and DDS regulations.
  • Collaboration with medical/nursing services within the home and community.
  • Develops ISA with team in compliance with DDS standards.
  • Maintains or encourages family/guardian/friends contact as appropriate.
  • On-call responsibilities for residential coverage and other related home situations.

The ideal candidate will possess time management skills; an ability to prioritize and follow through; strong interpersonal and communication skills; and a sense of humor.


Qualifications -
Bachelor’s degree in related field or at least two years of experience in a management level residential environment and hands on residential experience. QDDP & Supervisory status preferred with a minimum of 3 years’ experience working with intellectual disabilities. Possession of a driver’s license, excellent driving record and access to a safe, reliable, insured vehicle are required. Full time with benefits. EOE
