MCN is the respected leader of behavior health care in our community and actively partner other community providers. Competitive pay and benefits with average pay ranging from $53,000 to $65,000.

Case managers provide community-based outreach and are responsible for linking, coordinating, monitoring and psychoeducation. Our case management model utilizes a team approach with nurses, peer supports and paraprofessional staff providing comprehensive care. Positions are available working with children with serious emotional disturbance, or the adult population on either our team serving persons with intellectual/developmental disabilities population or the team serving persons with severe mental illness.

Requirements

· Bachelor's degree in social work with limited or full licensure in the State of Michigan or a bachelor's degree in a human services field and one year's experience with qualification as a QMHP, CMHP or QIDP.

· Possession of a valid State of Michigan Driver License (and ability to obtain Chauffeur License). Local travel is an essential function of this role.
