Case Manager - Outpatient - Warren, OH

Shift: Monday – Friday, 40 hours per week

Salary: $15-$18 per hour based on previous experience; Comprehensive benefits package

Responsibilities:

  • Perform and document psychosocial interventions with clients, family members and others.
  • Provide diagnostic assessments, individual and group interventions in collaboration with supervisors and/or other service providers, treatment planning, that are provided per best practice standards and meet program metric targets.
  • Productivity is achieved by submission of progress notes/billing logs. Productivity is defined as documented activity divided by hours paid. Must meet productivity expectations as one indicator of ability to adequately perform job duties.
  • Productivity/ impact ratio standard is 10 hours per week in contact with, or on behalf of clients.
  • Documentation is completed in a timely manner per agency policy. Documentation provides sufficient narrative regarding psychosocial and medical referral information to aid in delivery of services and to communicate the progress of the client.
  • Assessments, progress notes/billing logs and ITPs (and reviews) submitted within agency timeframes.
  • Will be constructive and serve to advance the quality of care, the mission of the agency, and the agency’s standing in the community.
  • Decisions pertaining to clients’ need for additional services, resources, or more restrictive settings.
  • Appropriate choice of interventions and techniques.
  • Pursues collateral contacts to gather additional psychosocial information when indicated. Utilizes supervision and/or consultation to mitigate risk and seek input regarding challenging circumstances.

  • Communicates effectively with other service providers throughout the agency, clients’ families and significant others, and with concerned members of the community.
  • Markets referral sources and maintains high level of customer service/satisfaction.
  • Participates actively and productively in agency meetings and supervision sessions.
  • Introduces and/or implements new ideas for improving the quality of care.
  • Attentive/responsive to feedback from clients, significant others, community partners/members.
  • Shares skills, knowledge, expertise with other agency staff (consultations, workshops, team meeting).
  • Advances own level of skill and competency through participation in continuing education opportunities.


Qualifications:

  • Bachelor’s Degree in social work or master’s degree in counseling is required.
  • At least two years mental health or community support experience is preferred.
  • Experience with dual diagnoses, counseling/case management, and/or community resource linkages.
  • Experience understanding and implementing various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery.
  • Ability to work effectively in collaboration within the community and with diverse client populations.
  • Must be able to provide coverage and to respond to emergencies on an on-call basis.
  • Experience with computers, EHR systems, office phone and cell phone, email, fax/copy machine.

Miscellaneous requirements: Criminal background check; Pre-employment drug screen; Clear driving record; TB test and First Aid/CPR. Periodic walking and standing during the day. May involve physical intervention with clients experiencing crisis situations. Potential exposure to blood borne pathogens. Strong social, communication, and written skills. Must have the ability to develop rapport and be perceived by clients and staff as approachable. Ability to multi-task, while maintaining accuracy and organization.
