ATTIC Correctional Services is currently recruiting for a full time Case Manager- Supervised Release Program. This position is responsible for the day-to-day case management of assigned clients through a statewide contract. Responsibilities include: reviewing client referrals, completing assessments, development and monitoring of case plans, participating in client staffings, completion of case notes and other documentation.

Duties include, but are not limited to:

  • Participates in pre-placement and post-placement client staffing’s in person (when possible) or via teleconference.
  • Completes pre-placement needs assessments and periodic needs re-assessments, as needed.
  • Develops and implements case plans based on Specific, Measurable, Achievable, Rational, Time-bound goals related to assessment results and client-specific risks and needs.
  • Completes staffing reports and case notes.
  • Transports client possessions between the institution and the community placement, as necessary.
  • Assists with the establishment and maintenance of a safe, appropriate client residence.
  • Makes referrals and coordinates services with multiple stakeholders and service providers.
  • Establishes collateral contacts for service coordination and continuity of care.
  • Meets with all assigned clients on a face-to-face basis.
  • Monitors services being provided through face-to-face contact and electronic communication to ensure clients’ needs are being met.
  • Reviews and approves client budgets and financial requests.
  • Attends monthly meetings with funder.
  • Other duties as assigned.
  • Regularly communicates with DHS personnel to ensure quality client service coordination and to mitigate safety and/or security issues.

Education & Work Experience

  • Bachelor’s Degree in Human Services or Criminal Justice required
  • Minimum of 2 years of experience providing community based Case Management serving one or more of the following populations:
    • Individuals who have committed sexual offenses
    • Individuals with serious and persistent mental illness
    • Individuals who have a developmental disability
    • Individuals who have committed violent crimes

Knowledge, Skills & Abilities

  • Valid Wisconsin driver’s license with good driving record.
  • Ability to travel frequently, 40-45%.
  • Demonstrated understanding and sensitivity to interactions with individuals from diverse backgrounds and cultures.
  • Excellent verbal and written communication skills.
  • Ability to interact with staff, clients, and other parties in an effective manner while maintaining professional boundaries.
  • Strong decision making abilities and self-motivated.
  • High level organizational, time management and planning skills.
  • Ability to maintain confidentiality at all times.
  • Demonstrated initiative and flexibility.
  • Experience enforcing rules, policies and procedures.
  • Proficiency in the use of automated computer systems and Microsoft Office software programs.
  • Ability to pass the Wisconsin Caregiver Background upon hire and annually thereafter.
