Summary: Provides direct nursing care to the residents while supervising the nursing activities for the assigned shift in accordance with the Nurse Practice Act, applicable State and Federal regulations, and Premier Living policies and procedures.

• Graduate of accredited school of nursing, LPN/LVN preferred.

• Valid LPN/LVN license in the state employed.
• Valid CPR certification preferred.

• One year experience as a supervisor in hospital, SNF or related health care setting. Must have training in rehabilitative and restorative nursing practices.

Summary of Essential Functions:

• Provide licensed nursing care to residents that is consistent with person centered plan of care.
• Communicate with primary care physician and IDT regarding status of residents, follow up with critical labs, order requests and physician notifications.
• Ensure all residents are treated with respect, dignity with consideration of their choices and rights including self-determination.
• Appropriate medication administration, ordering, and disposal, assessment of response and communication with physicians/ IDT.
• Ensure minimum staffing levels for nursing
• Ensure workspaces are tidy, safe and clean.
• Attend various facility meetings as required.
• Perform administrative duties such as charting, prepare reports, medical forms and billing charge sheets in accordance with policies and procedures.
• Appropriate compliance with infection control procedures and antibiotic stewardship.
• Participates in facility Quality Assurance Program (QAPI).
• Addresses and resolve resident/family/employee concerns.
• Pro-actively addresses survey and/or standards of care issues.

• Performs other tasks as assigned.
• Assures Pharmacy, dietary, physician consults are followed in timely manner.
• Verify Resident Census
• Ensure meet documentation and service requirements in accordance with facility policies as well as insurance and reimbursement providers.
• Participate in developing, maintaining, and updating facility policies and procedures that govern the day to day functions within the nursing department.
• Audit nursing clinical documentation and records as required.
• Participate in survey preparations and survey process as appropriate.
• Supervision of Certified Nursing Assistants.

• Skilled in direct patient care and highest qualithy care.
• Ability to communicate effectively with residents and their family members,and at all levels of the organization.
• Have strong leadership and problem solving skills.

• Ability to react decisively and quickly in emergency situations.

• Ability to organize and prioritize.

• Knowledge of directing/coaching/ education for nursing staff.
• Ability to maintain confidentiality.
• Knowledge of the Nurse Practice Act, state and federal regulations and Premier Living policies and procedures regarding nursing standards and delivery of care.
• Must meet physical, sensory requirements per policy manual.
