The Child Development Care Coordinator links families and children to developmental and behavioral services in the community. Overall, the Child Development Care Coordinator contributes to the CHS high performance culture by exhibiting our values and providing quality results that position CHS as the leader in delivering proactive behavioral health, case management, community and early childhood solutions for children and families.

Primary Job Functions

1. Conduct activities to educate and motivate physicians and other child health care providers to conduct routine child development screenings
  • Utilize a “Physician Champion” to engage health care professionals.
  • Conduct a minimum of one health care provider office-based education activity each month with physicians and other child health care providers.
  • Follow-up with at least one physician or other child health care provider each month who previously completed an office-based education session.
  • Market HMGA to physicians and other child health care providers through direct mailings.
  • Follow-up with office staff about how HMGA can assist with screenings, referrals, and follow-up.
  • Create and distribute a HMGA referral form at physicians’ offices.
  • Establish a system to provide referring health care providers with feedback.
2. Triage calls and make referrals to a variety of programs providing developmental and behavioral services.
  • Respond to calls and online portal entries from parents, caregivers and professionals pertaining to development, behavior or learning for children.
  • Evaluate caller needs, utilize resources and identify appropriate referrals.
  • Participate in the identification of service gaps and barriers.
  • Assist with maintaining developmental services database.
  • Collect data that includes verbal consents, parental and child information, care coordination activities, case closure and letters.
  • Conduct follow-up care coordination calls.
  • Support in the processing of completed developmental screenings.
  • Attend outreach functions or meeting in the community.
  • Provide information and referral services support to the Call Center.
3. Contribute to a positive, engaging work environment.
  • Develop a strong knowledge base and stay current on job-related issues and trends.
  • Participate actively in departmental meetings, training and education, as well as the quality process.
  • Comply with CHS’s code of conduct, policies, procedures and other obligations.
  • Assist with training other team members and providing back up when necessary.
  • Pick up projects on the fly; perform other duties as assigned from time to time.
  • Demonstrate the CHS Common Bond values in the performance of all job duties.


Job Qualifications

Education, Licenses & Certifications:

  • High School Diploma or GED equivalent, required.
  • Associate's or Bachelor’s Degree in Special Education, Early Childhood Education, Child Development, Social Work or a comparable Human Services field from an accredited university, strongly preferred.
  • Florida Driver’s License within 30 days from hire with daily access to a reliable and insured vehicle, required.

Experience:

  • Work experience involving young children, required.
  • Experience working with young children with special needs, preferred.
