Targeted Case Manager: Children's Community Action Team [Position # 410]

Our Children's Community Action Team (CAT) provides assistance to youth and young adults in SWFL to help them live successfully in the community. We link them to resources promoting a recovery-focused lifestyle within a holistic perspective. The structure of CAT is unique, and the results of their work are phenomenally impactful on the community.

POSITION DETAILS:

- Working as part of a team to coordinate treatment and supportive services to promote optimal community integration.

- Improving clients' functional abilities who have complex needs and providing appropriate case management services.

- Case management activities and monthly home visits promoting youth and family participation.

- Providing advocacy, referral, and care coordination services to promote cooperation between CAT and other local agencies.

- School advocacy and support to promote academic success.

- Linking responsibilities: researching resources and services to support youth and family needs, accessing/utilizing other services, school, vocational/employment support, applying for benefits or insurance.

- Fostering independent living and transitional living skills.

- Developing natural support systems for the youth and family.

- Thriving as part of a multi-disciplinary team.

REQUIREMENTS/FOR CONSIDERATION:

- Bachelor's Degree from a regionally accredited college or university in psychology, social work, behavioral science or related area of study = Required

- Youth/young adult/teen experience working with serious emotional disturbances, mental health, emotional and/or behavioral issues = Required

- Must obtain Case Manager Certification following three months of active employment = Required

- Knowledge of rules and regulations pertaining to care and treatment of mentally ill = Required

- Exceptional and intuitive track record in assessing client needs = Required

- Ability to work a flexible schedule based on client needs = Required

- Comfort with and compassion while relating to diverse backgrounds and cultures = Required

- Efficient, empathetic and evocative communication skills = Required

- Demonstrated and experienced-based client service abilities = Required

- Valid FL Driver's License clean driving record = Required

- Accurate reporting and record keeping skills = Required

- Skills in compassionately and effectively interacting with potentially aggressive individuals by de-escalation and diffusion techniques = Required

- Ability to exercise decisive and creative judgment in taxing or sudden situations = Required

- Microsoft Office 365 skills (Including Teams/Video Conferencing) and Electronic Medical Records experience = Strongly Preferred

- Staying compliant on all trainings and immunizations = Required

- Ability to pass a Level II DCF Mental Health Fingerprinting, Drug Screen and Local Criminal Check = Required

LOGISTICS & COMPENSATION:

- Full-Time Hourly/Non-Exempt Position
~ Target standard business hours Monday-Friday
