The Housing Authority of Kansas City, Missouri is seeking qualified applicants for the position of Choice Case Manager. The Housing Authority of Kansas City, Missouri (HAKC) has been funded by HUD to provide case management services to families who resided at Chouteau Court (CC), a family development that was demolished and the residents were relocated. The HAKC will provide intensive case management services in a five-step process:

Assess the eligible resident to determine service needs

Develop an individualized integrated care plan (ICP)

Refer and relate activities to help the resident obtain their needed services

Monitor and follow-up

Evaluate

The Case Manager will assist and follow the family for three years, which is for the remainder of the Choice Initiative Grant. As families were relocated from Chouteau Courts to other housing, the case manager will continue to follow the family and provide case management services.

To perform this job successfully, an individual must demonstrate the following competencies:

Ability to develop an Individualized Integrated Care Plan (ICP) including health, education, and employment needs for each member of the family

Ability to work well with other team members and other partner agencies who will be providing additional services in the areas of health, education and employment

Works with the resident on a day-to-day basis using professional judgement and discretion to implement the team determined ICP

Maintains a high level of ethical conduct regarding confidentiality, dual relationships, and professional stature

Must communicate effectively; verbally and in writing

Individuals must have the following qualifications:

Bachelor’s or Master’s degree in Social Work, Psychology, Sociology or other Human Services related field

Experience in case management

Experience with Microsoft Office and database entry

Reliable transportation

Cultural competence in working with low income and minority families is preferred.

Bilingual or experience working with immigration/citizenship preferred.
