Primary Job Objective: The Client Engagement Coordinator will support Healthy Connections Medicaid enrollment and will seek to increase approvals of Medicaid renewals for residents of Beaufort, Jasper, Charleston and Dorchester counties. The candidate will work to increase the efficacy of access to Medicaid benefits as well as care coordination of other available resources (including, but not limited to, SNAP, CHIP, Welvista, state and federal taxes, voter registration and other resources). Regular travel to assigned counties is required. Residents of Beaufort, Jasper, Charleston or Dorchester County, SC preferred.

Organizational Relationships: Works in the Engagement Department and reports to the Director of Engagement

Essential Functions include, but are not limited to:

  • Become a trained SCT Counselor
  • Learn to successfully navigate and utilize SCT web-based systems, including Thrive Hub, Monday.com, CharityTracker Plus system, and other systems used to perform job requirements
  • Assist beneficiaries in the completion of Medicaid applications and redeterminations both in person and via phone
    • This may include: multiple phone calls, case management, gathering supporting documents, and submitting with redeterminations
  • Maintain client confidentiality at all times
  • Serve as a liaison with Molina Healthcare of SC and SC Department of Health and Human Services to follow up on the beneficiary's status
  • Perform outreach and participate in community coalitions, work groups and task forces, initiate collaborative opportunities, facilitate meetings, facilitate and grow partnerships that would lead to Medicaid enrollment in all assigned counties
  • Establish and maintain partnerships with local Department of Health and Human Services and Department of Social Services offices, United Ways, businesses, community benefits organizations, nonprofits, technical schools and universities, schools, food banks, homeless coalitions, faith-based organizations, city and county councils and others in assigned counties
  • Maintain working knowledge of all SCT applications offered including administration of benefits and eligibility requirements
  • Conduct all SC Thrive trainings in assigned counties and statewide as requested
  • Attend and present at conferences, meetings, and trainings as needed
  • Work with Training Department to offer suggestions for enhancing trainings and training content
  • Attend personal development and leadership seminars as requested, as well as workshops, staff meetings, webinars, conferences, training refresher courses and meetings
  • Thrives in diverse environments, comfortable working with people from various socio-economic backgrounds
  • Other duties as assigned

Performance Qualifications

  • Education: Bachelor's Degree or commensurate work experience
