Minimum qualifications:

• Bachelor’s degree from an accredited institution in a field of study related to the work assignment.
• One year of experience working with seniors and/or facilitating programs.
• OR additional appropriate experience will substitute for the degree requirement on a year-for-year basis.
• OR additional appropriate education will substitute for the required experience on a year-for-year basis.

Responsibilities:

• Plan, gather residents and facilitate a diversified program of recreation that is designed to meet the needs of residents and/or the neighborhood.
• Develop a calendar with assistance from staff.
• Ensure programs include cognitive stimulation, as well as cultural, social, educational, recreation, physical, spiritual, psychological and creative endeavors.
• Provide 1:1 visits with residents as care planned.
• Complete daily documentation for resident participation.
• Conduct and assist with outings.
• Completes initial assessments, quarterly/annual reviews
• Write and review care plans
• Complete MDS.

The shifts covered by this position will include day shifts and some evening shifts.

Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado’s veterans. We strive to honor and respect those living in our community through companionship and person-centered care while providing a comfortable and safe place to live. At Fitzsimons, we provide long term care, short term rehab and have a secured memory care neighborhood and are conveniently located on the medical campus with UC Hospital and the Aurora VA Medical Center.
