We are looking for energetic, compassionate and self- starting individuals to join our friendly team. The Lutheran Home is seeking Full-Time Nurse Liaisonwho enjoy a fast paced environment with varied duties that include: -Provide information and referrals to initial inquires regarding residential, healthcare and community based programs and services matching qualified candidates to vacancies at a level that meets or exceeds budgeted census and maximizes census levels for all available accommodations while ensuring customer satisfaction; with an emphasis on short term healthcare. -Executes marketing efforts for all residential and healthcare levels of service and community based programs to maximize census levels. -Completes clinical assessments for all short term healthcare prospects and residential and healthcare prospects as directed. -Provides all services in accordance with the policies, philosophy and mission of Lutheran Life Communities. -Assists in the management of wait lists for levels of services as appropriate. -Ensures proactive and effective communication of critical information to the appropriate interdisciplinary team prior to admission to facilitate a positive move-in/start of care experience for each resident/patient and family. -Establishes, builds, and maintains relationships with professional referral sources; ex: community leaders and healthcare professionals, to facilitate qualified referrals and establish the organization as an asset to the market area. -Assists in the development and implementation of the written marketing plan including residential, healthcare and community based services which include analysis of community needs and competitor trends. -Work schedule may include working morning, evening shifts, weekends, and holidays.
