The Hospital Case Management Manager will work alongside Case Management Directors and Care Continuum Leaders to design and implement best practices related to case management. The Case Management Nurse Manager will collaborate with medical directors and senior hospital leaders to assess local resources and appropriate discharge planning. Moreover, the Case Management Clinical RN Manager will improve hospital metrics such as Average Length of Stay (LOS), Total Hospital Days, and Readmission Rates.
