CCP is seeking a motivated Clinical Pharmacist to work with our Medicare population. The Clinical Pharmacist will be charged with developing and maintaining a comprehensive program for high-quality and coordinated medication management through delivery of medication-related care to help achieve measurable outcomes and improve patients’ quality of life. The Clinical Pharmacist will collaborate with an interdisciplinary team to help achieve optimal patient outcomes.

This position will initially train in person and then will work remotely. You must live in the Capital Region of NYS to be considered as occasional on site work will be required.

Essential Functions include, but are not limited to:
Perform patient assessments to ensure the appropriateness of medication therapy and enhance patients’ understanding of their medications.
Monitor the effects of medication therapy.
Provide medication-related information to patients and clinicians.
Communicate with practitioners in a timely manner regarding new medications, new medication warnings and medication recalls.
Recommend laboratory tests necessary for monitoring outcomes of medication therapy.
Interpret data related to medication safety and effectiveness.
Document the care provided in patients’ records and update medication list as appropriate.
Identify barriers to medication adherence and work with patients to improve adherence.
Communicate with payers to resolve issues that may impede access to specific medications.
Identify cost containment opportunities.
Assess for opportunities to refer the patient to other members of the clinical care team including, social workers, case managers, care navigators, and diabetes or nutrition educators.
Perform medication reconciliation, patient counseling, transitional care services and patient and health care provider education.
Support practice transformation initiatives as part of a multi-disciplinary patient care team.
Collaborate with payors on value based care programs as appropriate

Requirements

PharmD Degree, completion of post-doctoral residency (PGY-2 preferred), or at least two years of clinical experience in ambulatory care. Enhanced credentials preferred (or willingness to obtain such credentials, i.e. BCGP, BCACP); Eligibility for licensure in New York State. Immunization certification desirable.
