The Clinical Social Worker LCSW will be a full-time, permanent employee for this growing health system that has provided exceptional patient care for over 80 years. This health system offers an array of inpatient and outpatient services, including a Trauma Center, Cancer Center, Family Birth Center, and Senior Care Center, and more.
Responsibilities of the Clinical Social Worker LCSW will include assessing a patient's psychosocial needs, developing individualized treatment plans, and providing external resources or referrals as needed. The Clinical Social Worker is also accountable for completing all written reports in a timely fashion, maintaining patient records as required by law and Medical center policy, and continuously cross-collaborate with multidisciplinary teams, case management, and external providers as needed.
