The palliative care Clinical Support Registered Nurse (Non- Remote) works as a member of the Interdisciplinary team telephonically assisting patients and families in managing the psychosocial, emotional and resource needs related to life limiting illnesses.

This position works as part of a small, closely connected unit along the lines of a busy provider office. The days and hours are M-F from 8:00 a.m. to 5:00 p.m. with little to no overtime, no weekends and no on-call responsibilities, although some on-call may be added as the office grows.

The hourly rate for the RN Clinical Support Nurse ranges from $26.75 to $34.00 depending on experience on internal equity.

The Clinical Support Registered Nurse (Non-Remote) Responsibilities include:

• Initiating contact with all Keystone patients living in home or assisted living environments as well as other patients/families as identified by the interdisciplinary care team.
• Assessing the physical and emotional needs of patients/families/caregivers related to the patient’s life limiting illness and environment. Participates in development, implementation, evaluation and updating of interdisciplinary care plans.
• Providing support and education for patients, families and community members regarding symptom management, disease process, patient needs, emotional and social aspects of life limiting illnesses.
• Assisting patients and families in identifying and utilizing appropriate resources to meet environmental, financial, legal or psychological needs.
• Assisting with directing patients and families to appropriate agencies to apply for SSI disability and Medicaid.
• Participating in goals of care discussions with patients/families as needed for patients identified as hospice appropriate.

Qualifications for the Clinical Support Nurse (RN, Non-Remote) include:

• Graduate of an accredited school of nursing and current Registered Nurse (RN) licensure in the State of Michigan;
• Two years of recent clinical experience is preferred. Homecare or hospice experience desired;
• Sound clinical judgment; and
• Knowledge and/or experience in pain symptom management with end of life care.
