JOB SUMMARY: Under direction of the Nurse Leader, provides direct care to residents to meet their daily needs and enable them to function at the highest possible level. While this care is provided under the leadership of the Nurse Partner (RN/LPN), the Care Partner is the residents’ closest advocate in the neighborhood. The Care Partner promotes the residents’ well-being through meaningful relationship building, and works with the neighborhood team to meet the social needs of the resident and of the neighborhood. The Care Partner plans and facilitates social activities for residents of the neighborhood which provide meaningful ways to spend time as recreation, exercise, relationship building and fun. The Care Partner also supports the neighborhood housekeeper in maintaining a clean, safe environment and in caring for the residents’ personal laundry. All Care Partner tasks are performed in accordance with current standards, guidelines and regulations, and in a safe and efficient manner.

  • CORE FUNCTIONS*
  • Attends to all resident personal hygiene and care needs, including but not limited to grooming, dining and nutrition, mobility, elimination, psychosocial and safety needs. Organizes all supplies necessary to complete the tasks.
  • Supports residents in self care, and provides assistance as necessary for those unable to provide for their own needs.
  • Follows universal precautions, proper infection control, sanitation and safety standards of practice in all work and activities.
  • Responds quickly and empathically to all residents’ requests for assistance and care.
  • Observes residents closely, identifying changing needs and conditions at first indication, and reports immediately to nursing or neighborhood leadership as appropriate
  • Works with all neighborhood team members, led by resident and family preference and Daily Pleasures to meet the needs and desires of all neighborhood residents
  • Continually seeks opportunities to meet residents’ psychosocial needs through both care planned and spontaneous actions

ESSENTIAL FUNCTIONS

  • Person-Directed, Quality Focused: Care Partners must develop an innate ability to observe and connect with residents in a way that gives them clear insights into each individual’s Daily Pleasures and concerns. Care Partners must demonstrate the intention to strive for high efficiency while always respecting and preserving residents’ personhood
  • Leadership: Care Partners must take an active role in shaping neighborhood life, not merely passively waiting for others’ instructions. They should see their contribution to neighborhood leadership as highly significant.
  • Critical Thinking: Care Partners should possess strong abilities to shape their practices and responses to specific situations around individual residents’ needs and preferences. Work with the Nurse Leader to find plausible solutions/responses that are resident-centered and appropriate. Care Partners should be assertive about sharing their own input, observations and suggestions with others.
  • Community and Team Builder: Care Partners should have strong team skills, both in terms of participating in team decisions and completing tasks in a team format. They should be skilled at facilitating residents’ involvement in community meetings. They should have a sense of belonging, as well as ownership, with regard to their own neighborhood.
  • Conflict Resolution: Care Partners should have a good understanding of healthy conflict resolution techniques, and demonstrate a willingness to work out any personal conflicts with others in a way that is in keeping with the neighborhood culture.
  • Interpersonal Sensitivity and Communication Skills: Care Partners should have a strong ability to communicate with residents clearly and respectfully, without patronizing or manipulating. Care Partners should be sensitive to the issues and difficulties that frail elders may face, both physically and emotionally. Care Partners should be able to clearly give feedback and ask for information from other team members and family members as necessary.
  • Dementia Sensitivity: Demonstrates an ability to facilitate the ability of persons living with dementia to enjoy daily life of the neighborhood in meaningful ways. Demonstrates an understanding of – and intention to avoid – care giving practices that detract from a person’s sense of personhood. Is sensitive to the particular challenges a person living with dementia faces in terms of communicating their needs and Daily Pleasures, as well as maintaining a sense of personhood, in the face of degrading cognitive abilities.
  • Interpersonal Sensitivity and Communication Skills: Relates well with residents and is able to motivate and empathize. Has strong ability to elicit residents’ input into shaping activities. Able to facilitate resident involvement to greatest degree.

ADDITIONAL RESPONSIBILITIES:

  • Supports all aspects of the neighborhood environment, including pets, plants, children, guests; facilitating all planned and spontaneous needs and activities.
  • Assists residents as needed to wash, fold and sort residents' personal laundry; supports residents in self care and provide assistance to ensure residents' personal laundry is maintained in quality condition.
  • Uses confidentiality and proper chain of command when passing along information, changes, or concerns.
  • Personally responsible and held accountable for following Fellowship Senior Living’s mission and values while maintaining and living out our philosophy of person directed care.

MINIMUM ACCEPTABLE QUALIFICATIONS:

  • Graduate of a state approved program for nursing assistants(CNA) or certified home health aide license (CHHA)
  • Received C N A certification with 120 days of employment
  • Competency evaluated, tested and registered with the state
  • High school graduate or GED preferred, but not required
  • Completion of cross training for neighborhood activities, neighborhood social service, neighborhood housekeeping, and neighborhood laundry required
  • Strong interpersonal skills necessary in order to communicate, cooperate with and respond effectively to the instructions and requests from a diverse group of residents, visitors and staff and provide information with courtesy and tact
  • Excellent written and oral communication skills in English in person and over the telephone
  • Must possess a strong work ethic. Must be dependable and have a clear understanding of the need for punctuality and limited absences, ensuring all members of the neighborhood are fully supported in their daily activities
  • Must demonstrate willingness and availability to work in the event of call-outs or other emergency situations (i.e. weather or other natural disaster) where overtime may be required
  • Ability to organize and prioritize numerous tasks and complete them with autonomy and accountability
  • Other duties as assigned. There are times based upon unusual work volume (increase or decrease), emergencies, or severe weather conditions, when it will become necessary for you to accept and perform a work assignment or assist with tasks which are outside of your regular duties or cross-trained assignment in which you possess the basic knowledge and skill to perform the assignment safely

WORKING ENVIRONMENT:
The Care Partner must be able to function independently, have flexibility, and demonstrate personal integrity when working with residents, families, the neighborhood team, and support agencies. The Care Partner may be in close contact with individuals who have infectious diseases and may be subject to the handling of harmful chemicals, infectious waste, unhygienic conditions, and potentially hazardous compounds, solutions, supplies, and equipment. The Care Partner must observe rigid, standardized guidelines to guard against disease, accidents, and other dangers by wearing personal protective equipment when needed. Requires physical activity and moderate movement throughout the workday in order to perform daily duties. Requires the ability to stand, walk, push, pull, carry, move, bend, kneel, squat, reach, stretch, and lift a maximum of 35 lbs. The Care Partner is often subject to stressful, uncomfortable, and emotional situations/individuals.
COVID-19 considerations:
The Pines requires all employees to be full vaccinated against Covid-19 and be able to show proof at the time of hire. Per New Jersey Executive Order 283, all employees working in a long term care setting must be "Up to date" on all vaccinations including any booster shot for which you are eligible per NJ department of health guidelines. Pines Village, in compliance with the NJ Department of Health, require all team members be vaccinated against the flu or have a valid medical exemption from your personal physician.
