- Physical – FREE OF COMMUNICABLE DISEASE STATEMENT (with-in 6 months) signed by MD/DO/ARNP/PA (NOT SIGNED BY CHIROPRACTOR)
- CNA License or HHA or PCT certificate
- Level 2 background screening with AHCA
- CPR card
- HIV/AIDS, Assistance with Medications and Alzheimer certificate
- Driver's license or photo ID and Social Security card
- If available: Auto insurance & Auto registration & Professional Liability Insurance & COVID Vaccine.
