HCA Solutions is currently seeking qualified candidates for the Community Health Organizer role at the Department of Health in Harrisburg in numerous different regions and is both office and remote work. Position is fulltime: 8a to 4p or 8:30a to 4:30p, 9a to 5 p with a 30-minute lunch break, is expected to start 6/12/23 and run until 10/31/23 with the possibility to extend. The CHO will work with community partners to identify vaccination sites, promote vaccination clinics, identify trusted messengers to promote vaccination and prevention tactics. Candidate will develop county coalitions that address immediate needs and future opportunities. As well as help community members access basic needs like food, PPE, mental health services, while providing updated information on mitigation strategies.

RESPONSIBILITIES:

  • Leads the identification, gathering, analysis, and reporting of health data used to develop program needs assessments.
  • Assesses performance of public health activities and identifies and recommends methods for improvement.
  • Oversees, revises, and approves reports describing program outcomes.
  • Prepares, oversees, or finalizes grant applications, program funding requests, and narrative reports.
  • Evaluates program achievements and identifies service gaps.
  • Assesses the underlying causes of health disparities so that targeted interventions can be developed and implemented. Recommends public health solutions such as screening, prevention, or intervention to address the social determinants underlying health disparities.
  • Develops initiatives with community partners to prevent or control public health threats.
  • Applies evidence-based practices to program policy planning and development.
  • Serves on committees, boards, or task forces, as assigned, as the program representative for the department.
  • Establishes and maintains effective working relationships with state health center staff, and local health agencies, providers, and governmental entities.
  • Performs related work as required.
Requirements:

Preferred: A bachelor's degree or two years of professional experience (and relevant community development, or activist experience)

  • Knowledge of non-professional health care services designed to improve the ability of individuals to perform daily living activities.
  • Ability to read and interpret written materials.
  • Ability to communicate effectively orally.
  • Ability to communicate effectively in writing.
  • Ability to effectively speak publicly.
