Position: Community Health Worker (CHW)
Location: Teays Valley
FamilyCare has an opening for a full-time community health worker (CHW) in the Teays Valley site. This position will assist high-risk patients with the care coordination in their home and community, serving as a liaison between patients and the clinical care coordination team.
There will be interactions with patient through weekly home visits, facilitation of communications between the patient and their provider. The CHW will apply social and emotional support, as well as, ongoing flexible, accessible support, as needs change.
Duties and Responsibility:
Works under the direction of the health care team bridging the gap between individuals, families, communities and health and human services.
Educate patients regarding available services within FamilyCare.
Qualifications:
- High School graduate (equivalent)
- Medical Assistant or Certified Medical Assistant
- Experience in computer data entry, medical terminology; proficiency in spreadsheet, database and word processing systems a plus
- Trusted member and live in the community being served (Teays Valley, WV)
- Interpersonal communication skills
- Ability to express empathy
- Counseling/health promotion skills
- Ability to read and write
- Knowledge of community resources
- Understanding of professional conduct
- Must have reliable, insured vehicle
- Must maintain valid and unrestricted driver’s license at all times
- May require travel to FamilyCare sites.
