BASIC FUNCTION:
Under the direct supervision and oversight of the Site Principal Investigator, Dr. Teletia Taylor, for the study entitled, “Reducing Inequities in Cancer Outcomes through Community-Based Interventions on Social Determinants of Health”, the Community Health Worker (CHW) will conduct distress screening and provide a connection between community resources and Howard University Hospital breast and prostate cancer patients
PRINCIPAL ACCOUNTABILITIES:
  • Assist patients in overcoming barriers in the post-treatment phase
  • Conduct distress screening for Howard University Hospital breast and prostate cancer patients
  • Provide linkages to community resources
  • Have monthly meetings with Principal Investigators from Medstar Health Research Institute, George Washington University and Howard University
  • Coordinate activities and ensure data quality and consistency at study site
  • Adheres to department policies, procedures, quality standards, and safety standards
  • Makes home visits as requested within established scope of duties
  • Participates in meetings and serve on committees and represents the research team in community outreach efforts.
  • Actively participates in community events and activities as needed
  • Performs other duties as assigned

MINIMUM QUALIFICATIONS:
  • High school graduation or equivalent
  • Computer experience and demonstrated competence with Microsoft Word, Excel, and the Internet
  • Must be able to travel and work occasional irregular hours as needed, including weekends
  • Must have a valid driver’s license
