POSITION: Community Health Worker

LOCATION: Red Cliff Community Health Center

WAGE: $15.00 – 18.00 per hour, plus benefits

SUPERVISOR: Community Health Manager

THIS IS A REGULAR FULL-TIME NON-EXEMPT POSITION

JOB SUMMARY: The Community Health Worker (CHW) is a trained frontline health worker who serves as a bridge between the community and service system. The CHW engages with community members across the lifespan to support them in the home/community.

DUTIES AND RESPONSIBILITIES:

  • Participate as a member of the Community Health, Covid-19 team which may include testing, contact tracing, and isolation support.
  • Participate in community health outreach activities:
  • Advise clients or groups on issues related to self-care, such as diabetes management.
  • Identify high risk members of target population with overdue health needs.
  • Distribute culturally appropriate educational materials such as flyers, brochures, or other material to inform members of the target population.
  • Attend community events to engage and strengthen relationship between community and health service system.
  • Collaborate with other service programs to assist clients with applying for social services, including Medicaid, Long Term Care, Foodshare etc.
  • Transport or accompany clients to scheduled health appointments or referral sites.
  • Provide feedback to health service providers regarding improving service accessibility.
  • The home visitor may be asked to help organize, staff, and/or facilitate certain groups.
  • Provide case management type services including periodic and on-going assessments, referrals to other resources and assistance with overcoming barriers to access.
  • Maintain strict confidentiality and safeguard privacy of patients in common areas.
  • Present a professional, caring image for the Health Center and its programs.
  • Maintain a cooperative relationship with other Health Center staff and employees.
  • Demonstrate tact, courtesy, and respect in communication and interaction with Health Center patients, visitors, and staff and with outside agencies and programs.
  • Promote a working environment noted for effective cooperation and collaboration between programs, services, and co-workers.
  • Advance job knowledge and skills through continuing education efforts with the approval of Health Center Administration.
  • Attend staff and other meetings, in-services, and other events as directed by supervisor.
  • Maintain a clean and safe physical environment. Alert administration and/or Health Center staff to problems and difficulties, as circumstances may warrant.

SUPERVISORY AUTHORITY: None

KNOWLEDGE:

  • Organization skills and attention to detail.
  • Computer skills; data entry, Excel program, Word, Publisher.

QUALIFICATIONS:

  • Minimum of high school diploma, HSED or GED required.
  • Current Certified Nursing Assistant certification, preferred.
  • Two years’ experience working with Tribal Community Health – preferred.
  • Current CPR certification or willing to accomplish within 90 days of employment.
  • This position must possess and maintain a current valid Driver License. Must be eligible to be placed on the tribe’s vehicle insurance.

PERSONAL CONTACTS: Considerable daily contact will be made with the general public, tribal leadership, Program Directors and staff, consultant(s), and the Tribal Community.

PHYSICAL REQUIREMENTS: The duties assigned to this position involve bending, stooping, lifting, and carrying. Items may be placed in overhead storage or shelving.

WORK ENVIRONMENT: Red Cliff Community Health Center; office and clinic settings. Exposure to hazards of the health care industry. All Tribal Buildings are smoke free.

TRAVEL REQUIREMENTS: The ability to travel and attend meetings and trainings; overnight and out of town, both locally & nationally.
