General Overview
The Community Health Worker (CHW) is a trusted member of the community who helps high-risk patient populations obtain better access to healthcare while coordinating their healthcare needs. CHWs have the skills and experience to understand what patients are going through, in order to help them navigate difficult circumstances. CHWs come from the communities they serve to act as caring neighbors who assist high-risk patients in addressing the social and medical problems that frequently result in poor health.
The Community Health Worker assists patients with complex medical and social problems to improve their access to and utilization of healthcare services. This includes working with patients on health goals related to Social Determinants of Health in order to help mitigate issues that serve as barriers to patients obtaining optimal health. While the CHW is not considered a clinical role, the individual must demonstrate the capacity to learn and apply basic clinical concepts in order to identify when a referral to a licensed clinician is appropriate.
The Community Health Worker works with high-risk patients who are receiving care at Salem Hospital. This high-risk patient population presents a unique set of challenges, including higher rates of illness and hospitalizations. These patients can often be harder to contact and engage with, in addition to having weaker ties to their community and services within their Primary Care practice. Many of these patients visit their local Emergency Department/Hospitals instead of a Primary Care physician, resulting in fragmented care.
The CHW's efforts are integrated into the Community, serving as a bridge between the medical providers and patients in the community that are disconnected. The CHW makes efforts to re-engage patients with their medical team to improve the overall patient experience and health outcomes. The CHW must make effort to develop trusting, working relationships with the patients they serve, while being supported by Care Coordinators and Social Workers.

- Preferred: High School Diploma
- Experience
- Required: Local community resident with good knowledge of the resources available within the community. Solid knowledge of the Core Competencies for CHWs, as identified by Massachusetts Department of Public Health. Knowledge of outreach methods and strategies. Health education for behavioral change. Community capacity building. Knowledge of special topics in community health.
- Preferred: Prior experience as a community health worker, health coach or outreach worker desired. Prior healthcare experience a plus.
