WVSU Metro Area Agency on Aging is actively seeking five regionally-based Community Liaison Specialists for Take Me Home (TMH) Transition Program. The Take Me Home (TMH) Transition Program is a program of the West Virginia Bureau for Medical Services (BMS). The purpose of TMH is to identify qualifying residents of long-term care facilities who wish to return to their own homes and apartments in the community and provide them with supports and services they need to do so.

Reporting to the Take Me Home Assistant Director, Community Liaison Specialists will be geographically located throughout the state corresponding with the areas covered by the five (5) TMH Transition Coordinators. The Community Liaison Specialists will be responsible for building positive working relationships with hospital and nursing facility social workers throughout the state by providing outreach, education and training about the Take Me Home Transition Program, including the TMH referral process, eligibility requirements and transition services and support.

Job Responsibilities Include (but are not limited to):

  • Manage caseload, documents, and applicant files electronically per HIPPA regulations
  • Generate referrals to the TMH Transition Program;
  • Provide outreach, education and training about the TMH Transition Program to hospital and nursing facility staff in their assigned region;
  • Establish a strong working relationship with hospital and nursing facility social workers and discharge planners in their geographic area of assignment
  • Work with hospital social workers to identify individuals who may qualify for Take Me Home and facilitate referrals to the Program for those who wish to transition to the community
  • Coordinate with state TMH Office staff, TMH Transition Coordinators, and TMH Housing Specialists to support TMH participants transition to the community;
  • Meet with residents upon their admission to nursing facilities to assess their interest in transitioning to the community, provide information about the Take Me Home Transition Program and facilitate referrals to the Program for those who are interested in transitioning
  • Report and track liaison data and activities to the TMH office
  • Maintain a contact list of hospital and nursing home staff

Desired Skills and Qualifications:

  • Have an understanding of the Centers for Medicare and Medicaid Services Money Follows the Person (MFP) Demonstration Program
  • Have an understanding of the Take Me Home (TMH) Transition Program
  • Have an understanding of the eligibility requirements and services provided by the Medicaid Aged and Disabled Waiver, Traumatic Brain Injury Waiver, and other Home and Community Based Services available in the state of West Virginia
  • Have an understanding of potential barriers to successful community living
  • High School Diploma/GED Required
  • Must have excellent communication and organizational skills, especially in handling sensitive information
  • Valid driver’s license and reliable transportation - job requires extensive daily travel in geographical area
  • Excellent time management skills
  • Proficiency in word processing applications and document management software
  • Ability to work collaboratively with other service professionals - social worker staff administrators, Transition Coordinators, discharge planners etc..
  • Must be able to work in both an office environment and in the field with minimal oversight

Pay: $35,000.00 - $37,000.00 per year
