Overview


We are looking for a Community Resource Advocate to join our family. Under the direct supervision of the Community Outreach and Advocacy Manage r, this position is responsible for providing patients with assistance in meeting their healthcare needs including enrollment in pharmaceutical charity programs, Medicare, ACA Health Insurance, preparing disability applications for patients through the SOAR program, providing education and completing paperwork for new uninsured patients, referring patients to Medicaid, and referring patients to other community resources based on their expressed needs. Additional duties include creating and maintaining relationships with community agencies serving the organization’s patient population through community outreach and engagement.

Community Resource Advocate Qualifications:


Minimum: necessary to adequately perform this job. However, any equivalent combination of experience, education, and training which provides the necessary knowledge, skills, and abilities would be acceptable, subject to any legal and/or regulatory requirements; Must be able to sit, stand and walk for long periods of time; Ability to read and understand the English language; Ability to effectively maintain confidentiality of records and communicate with all levels of personnel

Experience: Experience in a medical office setting required.

Bilingual (English/Spanish): Preferred but not required

Additional Required Skills: Knowledge of medical office software and comfort in using computers in daily work activity. Must have excellent verbal communication skills. Excellent organizational skills. Must be able to problem solve and have critical thinking skills. Understand and maintain patient confidentiality

Additional Preferred Skills: Basic medical terminology. Proficient in use of all computer software utilized in the organization, including but not limited to Epic, ACA software, SHIIP software, and TPC.

Education: High School Diploma or GED certificate required; Bachelor’s Degree preferred

Certification(s): SHIIP Counselor, SOAR Counselor, ACA Navigator, completion of TPC training


Community Resource Advocate
Key Responsibilities

1. Provide excellent customer service to all constituencies – community members, patients, fellow employees, community agency staff.

2. Utilizing motivational interviewing techniques, ascertain individuals’ current needs and resources.

3. Based on information provided by individual served, provide education about requirements for programs as well as program enrollment and/or referral.

4. In both internal and external interactions, uphold MAP, Medicare, disability, ACA insurance, FQHC, and community agency enrollment and program guidelines, policies, and procedures.

5. Participate in QI initiatives to improve processes and minimize errors and variation.

6. Work well with all appropriate staff to maintain efficient workflow.

7. Utilize data systems to update and communicate with staff in order to receive appropriate referrals and expedite patient processing

8. Adhere to all reporting, training, and certification requirements of SOAR, SHIIP, ACA, FQHC, and other community programs.

9. Recruit additional volunteers to increase capacity to provide SHIIP and SOAR services.

10. Document activities in The Connector, TPC and other means as requested by team leads and/or program manager.

11. Maintain positive relationships with community members, healthcare providers and community partners.

12. Develop and maintain positive relationships with community members and agencies through community outreach and engagement.

13. Physical demands include but not limited to setting up tables, chairs, booths and IT equipment at off-site enrollment events.

14. Perform other duties as assigned.
