Qualifications:

High school graduate or equivalent. Excellent verbal and written communication skills are required for this job. Strong organizational and problem-solving skills. The ability to be flexible and adaptive to change while maintaining a positive attitude. Ability to prioritize tasks, work under pressure and complete assignments in a timely manner. The employee must have the ability to operate computer systems. The employee must be friendly and personable and be able to work in a team-oriented environment.

Basic Function:

Responsible for linking patients with community resources.

Duties and Responsibilities:

• Provide assistance to patients and families that have identified needs, such as housing, transportation, food security, financial assistance and other required services.
• Collects data about patient through screening tools, case history, and/or referrals from practitioners; evaluates to determine proper approach and/or referral to other members of the social services team
• Provide patient screenings and enter appropriate data as it relates to the Accountable Health Communities project.
• Document activities and assistance provided as a follow-up from a referral or patient case in the electronic medical record.
• Participates in the social services team meetings.
• Maintains proper tracking logs and monthly reports to reflect patient contacts and progress.
• Maintains accurate and appropriate case records in the electronic medical record.
• Collaborate and coordinate with other members of the social services team, as needed, to respond to all needs of the patient.
• Seek out additional community resources that could benefit our patients
• Coordinate and verify interpretation services charged by third-party vendor. Other duties as assigned.
