SUMMARY:
The intent of the Community School Partnership Program is to align community resources to improve student and family outcomes through the integration of academics, health and social services, youth and community development, and community engagement. The community school model includes evidence-informed programmatic strategies as an effective approach to mitigate the academic and social impacts of emergencies that affect local communities, improve school responsiveness to student and family needs, and to organize school and community resources to address barriers to learning.
The Community School Partnership Program Supervisor is responsible for the overall operation of the Community School Partnership program, including the direct supervision of staff and the implementation of the specific components of each program. The Program Supervisor monitors the case management, service coordination and plans of the clients, especially the complex, high risk cases, and provides staff with specific program requirements. Work as a member of the interdisciplinary team which delivers the components of the program. The Program Supervisor will also support the Case Managers and provide coverage to the school sites and home visits, when applicable.
