PIBLY Residential Programs, Inc. is a Bronx-based, not-for-profit, voluntary organization certified by the New York State Office of Mental Health. It operates residential programs which provide rehabilitative and support services to adults suffering from mental illness. We Currently have openings for multiple Counselors (at least one being bi-lingual English/Spanish) at our Apartment Treatment Program (ATP) site. Apartments in this program are for residents who do not require 24-hour on-site supervision and have reached a level of functioning where need for staff assistance or training can be met through staff visits to the apartment.

PRIMARY TASK AND RESPONSIBILITIES

- Should be bi-lingual

· Maintain primary responsibility and accountability for assigned caseload of clients. “Primary responsibility” means provision of needed services within residential program setting

· Assessment of resident’s functional abilities and service needs

· Development of service plans, with residents’ input, aimed at assisting resident establish and accomplish goals

· Provision of case management including, but not limited to, necessary connection to needed/desired services (e.g. day programs, vocational rehabilitation services, legal services, medical/dental services, outpatient psychiatric services, etc.) and attending case conferences focusing on coordination of treatment/services provided to residents

· Assist/teach residents daily living/social skills including, but not limited to, budgeting, personal hygiene, maintenance of living space, shopping, traveling, meal planning/preparation, use of leisure time through participation in recreational and social activities, facilitating participation in available program groups

· Facilitate interaction with clients enabling the development of a therapeutic alliance

· Work with clients on discharge planning from date of admission

· Document all interactions with clients through progress notes

· Attend unit meetings as scheduled

· Attend weekly supervision with unit supervisor

· Report any changes in client’s behavior immediately to Unit Supervisor or Program Director

· Available to work longer hours when needed in emergency situations

· Follow all agency ethics and conduct

· Perform other necessary duties assigned by Unit Supervisor

· Participate in annual anti-sexual harassment training annually
