CRS Case Manager Job at Jewish Family Service of Colorado
Position Summary: Provides professional case management services to individuals and families with the goal of promoting stability and self-sufficiency as part of the metro-wide homeless prevention program. This is a Denver based position with flexible remote work options.
Salary Range: $21.00 -$30.00 per hour
Hiring Range: $21.00 - $25.00 per hour
JFS supports a flexible work environment with hybrid work options available to most positions. Colorado residency is required.
Responsibilities:
- Assists with the CRS rent assistance new client intake and eligibility screening.
- Provides Information and Referral to callers.
- Performs comprehensive needs assessments of adults, children, and families.
- Assists with client intake assessments and process.
- Develops, implements, monitors, and modifies treatment/service plans with clients.
- Provides support, resource navigation and supportive services to assist clients in reaching goals.
- Inputs client information into databases as required.
- Requests financial approval for services of the program based on assessment of need and availability of resources.
- Maintains accurate case records; documents client contacts and progress to ensure accountability.
- Participates in staff trainings as required.
- Completes or provides input to reports documenting program activities.
- Adheres to all grant eligibility and documentation requirements
- Uses creativity and innovation in program development and service delivery.
- Maintains confidentiality of client and Agency information.
- Assists with special department programs.
- May assist as needed with Weinberg Food Pantry clients.
- Collaborates with community prevention agencies.
- Other duties as assigned.

Requirements:Education: Bachelor’s degree in social work or closely related human service. MSW preferred.
Experience: Prefer 1-3 years’ experience as a Social Case Worker for a public or private agency.
Other: Knowledge and understanding of the Denver metro area resources is preferred. Bi-lingual in Spanish preferred. Strong communication, organizational and interpersonal skills. Ability to interact effectively as a team member and independently with Agency staff and with a diverse client base. Ability to multi-task in a fast-paced environment. Must be proficient in Microsoft Office.
