ABOUT THE ORGANIZATION
SaVida Health is a specialty medical practice providing outpatient opiate and alcohol addiction treatment services. SaVida Health's care model includes medical care, counseling, comprehensive toxicology testing, case management and medical management of psychiatric medications. SaVida provides compassionate and individualized treatment programs to support those with substance use disorder.
Summary:
At SaVida Health, the CSAC is responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered in an outpatient substance abuse treatment setting. The CSAC will be responsible for assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.
The CSAC will complete all relevant paperwork in a timely fashion as required by agency and licensing bodies; this includes but is not limited to: intake paperwork, psychosocial assessments and patient histories, needs assessments and problems lists, session notes, discharge and aftercare plans, collaborative forms and assessments, billing forms and insurance requests.
Role and Responsibilities
  • Ensure and maintain the confidentiality and safekeeping of all client records and information related to treatment
  • Maintain up to date knowledge of confidentiality statutes (HIPAA, 42CFR)
  • Maintain professionalism in workplace including professional boundaries and codes of ethics
  • Participate in weekly staff meetings
  • Participate in group supervision meetings as scheduled
  • Participate in clinical and administrative supervision meetings as requested
  • Participate in scheduled Performance & Quality Improvement Meetings (PQI) as requested
  • Ensure all agency policies and procedures as they pertain to client’s engagement in treatment are upheld
  • Develop effective working relations and cooperate with medical team throughout the entire case management process
  • Behave as ambassador in support of integration between behavioral and medical team throughout entire case management process
  • Establish a network of connections and effective relations with community agencies, hospitals, and service providers to effectively link patients with services that meet their needs and strengthen their stability.
  • Provide continuity of care as evidenced by documentation of and communication with collaborative providers and agencies
  • Act as client advocate to coordinate required services or to resolve emergency problems in crisis situations
  • Collaborate with therapist, medical providers, and other members of the care team to ensure patient needs are being met and monitor program compliance.
  • Maintain valid professional liability insurance, when applicable.
  • Perform other duties as requested
Qualifications and Education Requirements
  • Degree in counseling, social work or other allied mental health field
  • CSAC certification required
  • One-year experience working in the substance abuse treatment field
  • One-year experience working in the mental health counseling field
  • Bilingual/bi-cultural preferred
  • EMR/EHR experience (Athena software a plus)
Preferred Skills
  • Ability to work independently and as part of a team
  • Ability to pan and prioritize work with limited supervision
  • Working knowledge of the social services, justice, health, community service, and governmental agencies a plus
  • Serve as liaison to participants, monitors, employees, and professional liability insurance providers and provide advocacy as needed
  • Ability to communicate well, have the investigative know-how, and ability to work with individuals to may exhibit strong personalities
  • One-year experience using an Electronic Health Record. Previous use of Athena Electronic Health Record preferred
  • Computer literacy; typing skills, knowledge of EMRs, MS Word, MS Excel preferred
  • Ability to work with individuals and groups from diverse populations; bilingual/bicultural preferred
  • Excellent verbal and written communication skills
  • Ability to present information in a group setting
  • Ability to effectively manage time to meet deadlines
  • Proficiency in clinical documentation (assessment, treatment planning, progress notes)
  • Maintain and support the mission and goals of the agency
