SUMMARY:

As a Teacher/ Counselor at LifeWorks, Inc you will be a direct support professional (DSP) supporting people with intellectual and developmental disabilities in a residential program. You will be working in a beautiful home in the suburban town of Wrentham, MA, and receive a Staff Retention Bonus shortly after joining our team.

All-female residents which would need direct personal care!

Job Responsibilities: (Case Management duties) & (Direct Care Work)

• Tracking and recording ISP data
• Writing Monthly progress notes
• Attending ISP meetings
• Completing ISP assessments
• Assisting with appointments and all follow-up
• Serving as a contact for other providers (Guardians, Day Program staff, etc.)
• Cooking, assisting with feeding, etc.
• Hands-on support with toileting
• Showering
• Changing briefs, and colostomy bag if needed!

• A Bachelor's degree or an associate degree in a related field with two years of experience, or five years of related work experience in Human Services required.
