We are looking for a compassionate Caregiver to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan.

TITLE OF POSITION: Direct Care Worker

POSITION RESPONSIBILITIES

Follows the service plan to assist the consumer in maintaining a healthful, safe environment.

Performs functions specified for each individual consumer on the service plan.

Receives written instructions from the supervisor.

Has knowledge of Agency policies and procedures.

Is oriented and trained in all aspects of services to be provided to consumers.

Assists persons at home in managing and maintaining a household, bathing, dressing, meal prep and incidental household tasks.

Will assist consumers with the following activities:

-Housekeeping

-Assist patient with eating and meal preparation

-Arranging for transportation to and from appointments and accompany client to appointments

-Providing or arranging for social interaction

-Shopping

-Companionship

Reports any change in the consumer's mental or physical condition or in the home situation to his/her immediate supervisor or Agency Manager.

JOB CONDITIONS

The ability to drive and the ability to access consumers’ homes which may not be routinely wheelchair accessible are required.

Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the consumer's condition and to perform consumer services.

On occasion, may be required to bend, stoop, reach; lift and/or carry up to 50 pounds.

Must be able to communicate clearly, both verbally and writing in English; understand and carry out directions and instructions, record messages and keep simple records.

QUALIFICATIONS

1. Completion of at least a High school diploma/ GED.

2. Must have completed an Agency orientation/training & competency test.

3. Must be in good physical health; free from health problems that may be harmful to consumer, self and co-workers and must present appropriate evidence to substantiate.

4. Must understand and respect consumers, including ethics and confidentiality of service and have a sympathetic attitude toward providing services for consumers.

5. Must have maturity, emotional and mental stability and experience in home services or homemaking.

EQUIPMENT OPERATION

Hand washing materials, Hoyer lift, wheelchair and house cleaning products.

Pay: $13.00 - $15.00 per hour
