Trinity Assistance provides services with our Direct Service Professional's whether it is assistance needed attending a community class, assistance with grocery shopping, or even meal prep, our staff are ready and willing to assist. Supports and services are tailored to be person-centered and designed the way our clients want it.

• Qualifications and Education Requirements: High School Diploma / GED The ability to lift to 50 lbs., stoop, bend, reach, grasp, listen, walk and speak are required.

Must have a Valid New York State driver's license & Dependable vehicle Must be 18 Years or older.

Preferred Skills: Previous experience working with people with intellectual and developmental disabilities preferred.

Key Responsibilities: Assist with social skill building in the community by promoting appropriate social etiquette/interactions and communication skills through role modelling and redirection.
Assist the person supported by aiding and training in all areas of daily living, including, but not limited to the following:
Laundry and care of clothing
Cooking, nutrition, dietary needs, meal preparation, menu development and grocery shopping
Household maintenance and cleaning
Budgeting, banking, and comparative buying
Assist the person supported in maintaining good good health and safety by following the guidelines set forth in the Life Plan.
Assure the safety of the person supported in the home and community
Teach safety precautions in the home and community
Conduct monthly fire drills
Notify Trinity Assistance of emergency/health and safety concerns
Promote awareness of traffic signs and “stranger danger” Assist the person supported in maintaining and carrying personal identification and learning emergency contacts

Completes written documentation of Daily Service Notes, Time Sheets, Reimbursement and Mileage claims according to guidelines, etc.
Reports on all incidents, illnesses, or other unusual circumstances according to policies and procedures of Trinity Assistance and OPWDD.
Attend all required training and other meetings such as Life Plan Meetings.

Other: Direct Service Professional  will be an active member of the household, and community; including but not limited to the following:
Assisting the unique capacities, personality and potential of the person supported
Getting to know the person supported through assessment and discovery
Promoting advocacy with the person supported
Developing professional relationships
Exhibiting professional behaviors
Showing respect for diversity and inclusion
Creating detailed, clarified, spell checked and correct documentation records
Education, training, and self-development activities
Exhibiting ethical behavior on the job
DSP will ensure monthly and annual training is completed by the due date

Job Types: Full-time, Part-time, Temporary

Pay: $13.20 - $20.00 per hour
